To relieve those in need by reason of age, ill-health, disability, financial hardship or other disadvantage, particularly older people and adults living with dementia, poor mental health, loneliness, isolation or learning disabilities, residing in South Aberdeenshire (Scotland), by providing practical, social and emotional support services to enable them to live independently and well in their own homes and communities. Such support may include (but is not limited to): Home visiting and practical assistance (for example, shopping, attending appointments, home organisation and cleaning support). Social companionship and isolation prevention. Telephone befriending and wellbeing checks. Assistance in maintaining connections with family, friends and community groups. Signposting and liaison with other care providers to meet identified needs.
