Core program provides supportive housing and outreach case management program to consumers of mental health services in the regional municipality of halton - appointed lead agency for the halton homes program and entered into a partnership to provide housing and case management services for consumers of mental health services who are homeless or at risk of being homeless. - assertive community treatment team (actt) provides assertive clinical and treatment services to people with persistent serious mental illnesses - justice supportive housing program provides supportive housing for people with mental illness who have come into contact with the justice system - health and wellness program provides social recreation opportunities including a variety and leisure activities for consumers of mental health services
