Establishment and operation of an ambulatory surgical centre at 9355 leslie street richmond hill ontario. The plan is to situate: (a) operating and procedure rooms recovery rooms surgical support rooms and reception on the second and third floors; (b) a diagnostic imaging clinic on the first floor and in the basement; (c) an eye surgical unit on the first floor; (d) primary care and specialist health clinics on the fifth floor; and (e) other support facilities including a medical device reprocessing department; pharmacy and storage on the sixth floor and (f) staff amenities on the fourth floor.
