Scg currently operates five programs for specific populations of people facing an intersection of barriers. Sanctum 1.0 established in 2015 as saskatchewan's first hiv hospice and transitional care home is a 10-bed residence with 24-hour home care staff and nursing staff that provides holistic wrap-around care and support. The hiv aids response team ('hart') program established in 2017 is an interdisciplinary team that links complex clients in hospital care to continuing care in the community. The sanctum 1.5 program was established in 2018 and is canada's first hiv and high-risk prenatal home providing supportive services to pregnant women living with hiv or at high risk of contracting hiv while pregnant and at risk of having their infant being apprehended at birth. The prenatal outreach and resource team ('port') was established in 2021 and is a case management team that provides integrated wrap-around support to women living in the community who are pregnant and at risk of having their child apprehended at birth. The 2nd steps program established in 2022 provides second stage housing for graduates of sanctum 1.5 and their children and rapid housing for women in port who are pregnant and homeless. Sanctum has 4 suites in our 2nd steps apartment building (1114 ave w north) that are now dedicated assessment suite units. These units are fully furnished and ready for families to move in to begin working on their case plans effective march 1 2024. Sanctum assessment suites in a program for parent(s) who have lost custody of their child(ren) and/or at risk of losing custody of their child(ren) and are needing an assessment to take place to determine capacity in a range of different risk areas. Sanctum assessment suites will provide parent(s) the opportunity to parent their children while assessment takes place and will allow families to stay together while they transition to a longer-term living arrangement.
