Assisted living - psws provide personal care & home support to physically frail older adults at richview residence and a 2 km radius of this building (hub & spoke model). Care managers coordinate the program and offer information and referral to other community services emergency response technology - provides a quick response from onsite staff for personal & potential urgent care situations. Short stay transitional care unit - for individuals who reside in the geographical community to bridge care needs from hospital before returning home or waiting for ltc placement. Congregate dining - structured social & meal program provided for older adults of richview residence twice per week. Care manager coordinate all programs.
