Rexdale community health centre delivers integrated coordinated clincal services through a team of physicians nurse practitioners nurses dietitians social workers a physiotherapist and a chiropodist. Our multidisciplinary team addresses the health needs of individuals and families at all ages and stages of their lives. In collaboration with the city of toronto's public health department rexdale chc also offers dental services for children students and seniors.
