.anticoagulation (inr) service: point-of-care monitoring and dosing management .clinical support services: central point of contact for healthcare providers to help complex patients in a vulnerable state navigate the health and social services .anxiety workshop: helping participants understand anxiety and its impact on life. .blood sugar basics: promoting and teaching lifestyle choices to help manage blood sugar .deprescribing service: assisting patients to safely stop or decrease the dose of medication they no longer need with the help of the pfht pharmacist in collaboration with family physician. .fht to quit: smoking cessation with team approach to meet the participants quitting goals .footsteps to parenting: a support group for new parents with babies younger than 6-months of age. .heart health: promoting and teaching lifestyle choices to help manage cholesterol and blood pressure .introduction to sleep & insomnia: teaching strategies to help those with insomnia or poor sleep. Requirement for 6-week program. .mindful eating: exploring eating habits and the relationship with food and strategies to make lasting changes to food choices. (insufficient room on form to include other programs) being with chronic pain: becoming mindful with chronic pain and understanding the links between pain mood and function. - talking about medications: providing participants with tools and support to become involved in decision making with medications.
