Leeds & Grenville Community Family Health Team

The leeds & grenville community family health team is comprised of physicians nurse practitioners registered nurses dietitians other interdisciplinary health care providers and management and support staff. They provide a full range of health care services to the community of leeds & grenville. Programs provided include health promotion disease prevention diabetes education chronic disease management and comprehensive primary healthcare services.
