About Guelph Community Health Centre
The guelph community health centre delivers a range of programs and services focused on improving the health and well-being of our community by providing services that eliminate barriers to good health for the prioritized populations we serve; using a health equity approach to care. The equity-deserving populations we serve include homeless/ under housed those with moderate to serve mental health and addictions new canadians and refugees low income lgbtq+ community members indigenous community members and vulnerable children and their families. Our core program is our delivery of wraparound primary health care services including physician and nurse practitioner-led patient care with comprehensive interdisciplinary supports including social work dietitians and client navigation. Community-based services include our food security-focused seed program parent outreach worker services within priority neighbourhoods our peer-led breastfeeding programming chronic disease self-management programs and peer-leader programs as well as out street outreach services( including specialized outreach services and welcoming streets along with outreach nursing integrated within the shelter system). The guelph chc is also a service delivery partner for earlyon services which include drop-in play-based learning opportunities focusing on early childhood development and caregiver supports
