To improve the lives of people with intellectual disabilities who are challenged to find services to meet their needs by providing person-centered individualized supports. Fairfax was founded to provide services to individuals who are considered to be 'in crisis'. The organization primarily works with manitoba's department of families (community living disability services program) but also with local school divisions and other funders and referrers to establish and provide supports to individuals who are challenged to find services elsewhere. Funding and regulation is provided under a provincial service purchase agreement. The organization reviews referrals assesses individuals' support needs submits proposals detailing support services to be provided and funding required provides the indicated services in response to changing circumstances in supported individuals' lives. Support models in use at fairfax include shift-staffed supports in licensed residential care homes home shares (like foster care for adults) supported independent living individualized day services and outreach supports to individuals making transitions fro institutional medical or correctional settings. In addition to trained professional support staff team leaders and case managers fairfax provides in-house specialized behavioural consulting and contracted nurse practitioner supports as needed.
