1) deliver nutritious hot and frozen meals 2) provide transportation social interaction and a nutritious meal in a congregate setting 3) provide volunteer escorted transportation to medical/social appointments 4) arrange for brokered helpers to assist clients with home maintenance and home help 5) match clients with friendly visitors 6) offer regular telephone reassurance contact 7) conduct foot care and blood pressure clinics 8) refer clients to lifeline personal emergency response service 9) rent medical equipment 10) provide information and refferal service 11) provide wheelchair accessible transportation to county and city clients 12) settle clients in their homes safely and comfortably after a stay in hospital 13) offer services in conjunction with ontario at home and peterborough regional helath centre for a specified period of time at no cost to a client to support their safe transistion from hospital to home 14) offer exercise and falls prevention programs throughout the county and city 15) acess to primary care 16) grocery delivery program
