Community Care Durham

Community Care Durham (CCD) is a multi-service registered charitable organization that provides meal services home support respite adult day program assisted living supportive housing exercise and falls prevention classes and COPE mental health services for adults and their caregivers who have needs related to aging physical and/or mental health. Home management services (Enriched services) is a collaborative effort between CCD and Lakeridge Health to support the needs of high risk senior patients/clients who can safely be discharged home with a sustainable level of assistance administered through various service locations across Durham Region. CCD is committed to 'Supporting People Strengthening Community'.
