Community Care City of Kawartha Lakes is funded by the Ministry of Health providing access to primary health care and community support services for residents with barriers to healthcare. These programs include: Geriatric Assessment & Intervention Team Adult Day Care Diner's Club (social & physical activity sessions in communities). Elder Abuse: provide elder abuse education advocacy & assistance in collaboration with other local services. Friendly Visiting: giving emotional support & companionship through home visits and phone calls Respite Care Supportive Housing and Assisted Living for High Risk Seniors: caregiver relief in clients' homes on-call personal support for seniors to live independently. Meals on Wheels: deliver hot/frozen meals to clients Transportation: provide transportation services to elderly with special needs for medical social and recreational purposes including nonemergency & wheelchair transport Home at Last & Home First: assisting client in transition from hospital to home Hospice: bereavement and palliative supports Community Low Income Dental Clinic Cooking classes led by registered dietitian: focus on health eating Homemaking: housekeeping services for seniors
