Bellwoods offers clients non-medical support services accomodation and independent living education through a spectrum of services that include supportive housing personal support (attendant outreach) services transition programs and a variety of educational services and activities. Bellwoods provides both permanent supportive housing and transitional (short term) accommodations. Our supportive housing and transitional (short term) accommodations. Our supportive housing programs are provided at two dedicated supportive housing projects owned and operated by bellwoods [located at 300 shaw street and 1082 dundas] and provides services and programs in five additional integrated apartment settings located at: 25 elm street (elm street) 1 summerhill road (mimico apartment coop) 5 deauv ille lane (deauville place) 180 sudbury street (sudbury project) 835 birchmount road (walton place) 200 madison (madison view site) clients residing in our supportive housing units receive support services and assistance with activities of daily living delivered under the client's direction. Services include: lifts/transfers and positioning bowel and bladder dressing and undressing skin care respiration (ventilator bi-pap; cpap) personal grooming and general hygiene laundry eating and light meal and drink preparation (including g-tube feeding). Our transition programs provide short-term supportive housing for individuals transitioning from hospital at our reintegration care unit located at 300 shaw street.
