Public Education – presentations and events that educate the general population on Alzheimer’s disease and dementia Resource Lending Library – offers the use of books and other learning material Education – First Link Learning Series provided to people living with dementia and/or their care partners Caregiver Support – information education and counselling provided to care partners of those living with dementia. Offered in both individual and group formats In-home Respite – supervision and activation for the person living with dementia provided in an individual setting in the person’s home / provides respite for the care partner Social / Recreational Activities – several types of group activities are offered to the person living with dementia and/or their care partner (Minds in Motion/Exercise program Memory Café/Social Outings Art Gallery Group Activities Social Luncheons etc.) Mindful Music – individualized music playlists are created for people living with dementia for use in minimizing responsive behaviours and as a method for socialization Friendly Visiting – volunteers visit with the person living with dementia in their home and offer a social visit. This may or may not include the use of the Mindful Music program Behavioural Support – system navigation for Behavioural Supports Ontario (BSO) in the Sarnia-Lambton area. Supports people living with dementia in Long-Term Care who exhibit behavioural responses and provides support and counselling to care partners of these individuals First Link Care Navigation – system navigation for people living with dementia and their care partners to help connect them to appropriate information and supports Adult Day Program – supervision and activation for the person living with dementia provided in a group setting / provides respite to the care partner Finding Your Way / ID Registration – assistance with registering the person living with dementia with the Sarnia Police Service or Lamb
