Alzheimer Society Lanark Leeds Grenville

We provide support and education to caregivers and persons with Alzheimer disease and other dementias both in person via email and telephone. We provide weekly 8 different support groups for individuals with dementia as well as 10 support groups throughout Leeds and Grenville for family friends and neighbours. We provide education to the general public in schools health facilities such as LTC and Hospital to service and social groups and to other health care providers. We participate in health fairs and maintain a website facebook page to educate the community. We maintain a current library with a variety of resources and support First Link training. We participate in SAFELY HOME a registry for individuals with dementias that wander away from their homes. We fundraise to support our many programs and services including Art Therapy a men's cooking group and other activities which provide respite for caregivers and meaningful activities for our clients with Alzheimer disease and other related dementias. We have expanded our support groups throughout the counties as well as providing educational opportunities for clients in rural areas. We are members of a Memory Clinic Team imbedded in a primary health care team (the Upper Canada Family Health Team) which serves over 22 000 patients in and around Brockville. We are working with other Family Health Teams to expand our Memory Clinic activities at the primary care level across the counties.
