Acclaim Health and Community Care Services

Our alzheimer support services consist of day programs for adults with dementia or related diseases in oakville and burlington as well as caregiver support and education. We offer a variety of support programs to the community including friendly visiting programs (including memory visiting and telephone reassurance calls) hospice visiting and bereavement support - all of which depend heavily on the work of volunteers. We also offer in-home nursing care including the following: palliative care continence management enterostomal therapy and wound care. Our home support services provided through personal support workers (psw's) include: bathing skincare dressing feeding and supervision. We also mentor and educate service providers in hospice palliative care in halton peel and dufferin regions. We also administer falls & exercise and step-up programs for seniors.
