The organization provides supportive housing services to adults with physical disabilities in the region of hamilton wentworth halton and peel. We also have outreach services and a homemaking program in the hamilton wentworth area. Our binbrook location includes an occupational therapy department as well as transportation services. In addition we have programs that includes community home support for seniors in home care and support for individuals transitioning from hospital transitional beds that serve patients whose acute treatment phase is complete but who still require supervision personal care congregate dining and 24 hours access to support services including: assistance with activities of daily living mobility assistance medication prompting just-in-time support for urgent personal care and scheduled and unscheduled personal support services. Transitional care program-willett site brant community health program operates a 15 bed transitional bed program. This program will provide a temporary restorative and safe setting for individuals who are medically stable ready for hospital discharge but who are not ready or are unable to return home. West halidmand general hospital with care model provided by ableliving and high intensity supports at home program. This is for the delivery of home and community care services to ensure capacity exists to reduce overcrowding in hospitals long-term care homes and other congregate settings.
