Your Best Life Disability and Health Services Ltd (YBLDHS) supported people by simplifying and providing disability, health, and human services so they can live their best lives. We provided services across the life stages as our clients grow from infancy to adulthood as their needs changed. YBLDHS is recognised for our strong clientcentred approach. We also worked with key stakeholders and partners to ensure integrated and streamlined service pathways that facilitated access and overcame barriers for our clients. YBLDHS is quality assured, accredited, NDISregistered, valuesdriven, and invests in innovation and fresh ideas. YBLDHS provided services in the Sunshine Coast, Noosa, Gympie, and Moreton Bay Local Government Areas. The 20232024 financial year marks the thirtyfifth year of service delivery in the regional community since the organisation formed in 1989 and a similar level of aggregate revenue ($10,756,209) to that achieved in 20222023 ($10,750,150). During the 20232024 financial year, YBLDHS continued diverse service provision via a total of six business locations across the Sunshine Coast, Noosa, Gympie and Moreton Bay Local Government Areas including: Your Best Life Children s and Teens Therapy (YBLCATT) provided tailored multidisciplinary allied health therapy services to support the development and wellbeing of children and teenagers. Your Best Life Allied Health (YBLAH) provided specialised therapeutic services for adults including speech therapy, occupational therapy, physiotherapy and assessments. Your Best Life Mental Health (YBLMH) provided mental health supports, including psychology, specialised behaviour support, and assessments. Your Best Life Positive Behaviour Support (YBLPBS) provided specialised behaviour support and assessments. Your Best Life Employee Assistance Programme (YBLEAP) supported both employers and employees with services including counselling, psychology and wellness programs. Your Best Life Independent Living (YBLIL) provided tailored disability support services including Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Individualised Living Options (ILO), Short Term Accommodation (STA), High Intensity Support (HIS) and social and community participation. Your Choice Plan Management (YCPM) supported NDIS participants with personalised financial intermediary services and ensuring sameday invoice processing to allow them to focus on their goals.
