We assist older residents to navigate the aged care system so that they have an opportunity to access supports to enable them to age in place, in the community where they live. This involves helping them apply, attending various assessments, taking them to medical appointments and staying connected with them throughout the process, which is very confusing for most of them. We also hold information sessions in the community to increase awareness.
