We the organization help Seniors within the community to understand what HCP does and how to use it, help Seniors to get assessed by Aged Care Assessment Team/ACAT, and how to obtain HCP and be serviced at their homes. We do regular Day care and activities at Council's rent places. We have at least 255 seniors who are over 65 years of age and need HCP funded home care service. We help seniors to get the information on how to apply HCP by our social workers and volunteers . We also give or provide Home Care/ In home services who are either referred to us by My Aged Care and are assessed by this Charity or had been serviced by other HCP providers and this Charity's social worker and / or registered nurses.
