The organisation has pursued its charitable purposes by: Providing well integrated, multidisciplinary patient centered medical and allied health care; Providing support to its patients including those with, or at risk of, chronic diseases(s) with a strong focus on patient selfmanagement; Responding to the local community needs and priorities of the Barwon and South West of Victoria regions including the needs of Aboriginal and Torres Strait Islander people; Providing a range of accessible, culturally appropriate and affordable medical and allied health care to its patients; Supporting preventative medical and allied health care including promotion of health lifestyles, by addressing risk factors and lifestyle modification to prevent chronic disease and improving early detection and management of chronic disease within the local community; Offering high quality best practice medical and allied health care in accordance with industry accreditation standards including accreditation against the RACGP Standards for General Practice (3rd Edition) as mended from time to time, and accreditation for training where applicable; and Supporting the future primary care workforce including students.
