Our vision is to support communities where everyone can realise optimal health and wellbeing. We do this by delivering excellence in care for patients whilst responding to community needs in our disadvantaged catchment. In addition, we create change through our advocacy for the needs of our migrant, refugee, frail and mental health patients whose chronic conditions and poverty result in more complex care needs. Finally, we engage in teaching and research to disseminate our models of care to other high need regions.
