Heart Support Australia Limited

This year Heart Support Australia increased our peer support program delivery both within our hospital patient visitor program and our monthly peer support groups nationally. With nearly 30 peer support groups nationally, and 12 hospitals under agreement where we provide inpatient support our programs are reaching more than 10,000 patients per annum and helping those transition from hospital to home and support those with heart disease to improve their lives both physically and mentally.
