Adelaide PHN was established by the Commonwealth to reshape the delivery of primary health care across the Adelaide region. Our activities are aimed at improving health outcomes in the community, keeping people well and out of Hospital. APHN is engaged with community members who provide input on strategic plans and feedback to the organization. APHN works in collaboration with other health providers in both the community and tertiary sector to improve patient pathways and models of care. APHN supports all primary health care professionals, assisting with access to education and training and ensuring that there is key information about chronic disease states, different models or care and referral pathways and resources
